REQUEST FOR AUTHORIZATION TO TEACH:

INSTRUCTION

Instruction may be in classroom or offered through electronic transmission ("Internet") or distance education ("correspondence course").

REPORTING REQUIREMENTS FOR LICENSED SCHOOLS

A completion certification shall be provided to each student. For this course, student certifications are not reported to this Office by the schools.

Course certifications shall be filed with this Office electronically within 5 days of completion. [Upon licensure, the school is issued an ID# and password to add reports on-line].
