To request a registration package or printed licensing forms and information, send your e-mail request to
electronic_licensing@fldfs.com and be sure to include all of the following information:
Name
Mailing Address
Phone Number
Type of Registration Package (or list individual forms) *
* There is a registration package for each license type that contains an application, supplemental instructions and forms where appropriate.
You must clearly specify the Type of Registration Package in your request in order to receive the correct documents. If you are ordering individual forms or instructions, include the complete name of each one.
