Submission of the Canadian Dealer Notification form (OFR-CAN)
Uniform Consent to Service of Process
Evidence of a current registration as a dealer in the jurisdiction
in which the dealer's main office is located
Evidence of current membership in a self-regulatory organization or stock exchange in Canada
Effective 10/1/06, Florida Exempted Associated Persons
of Canadian Securities Dealers from having to Register pursuant to Chapter
517, Florida Statutes
